Care Coordinator visits your Home
Once your consultation is confirmed, your dedicated Care Coordinator from Our United Hearts Home Care will visit your home to complete a comprehensive in-home assessment. During this visit, we carefully evaluate care needs, understand daily routines, and discuss personal goals and preferences.
Based on this assessment, a customized care plan is developed to ensure every service is intentional, supportive, and tailored to your unique situation. Your Care Coordinator arrives prepared to listen with compassion, answer your questions, and provide clarity about next steps — laying the foundation for trusted, high-quality in-home care.
